Yes, you can provide the Trust with a disk containing the injured parties names, Social Security numbers and the corresponding claim numbers assigned by your law firm.
I received a letter stating that I did not file a Proof of Claim. What information do I need to provide so the issue can be resolved?
You were required to file a Proof of Claim by the Bankruptcy Court on or before the established Bar Date of March 15, 1996.

Submit written notification requesting the claim to be withdrawn.
What happens if the injured party died after the claim was filed?
The Trust needs to know the identity of the person who will act on behalf of the deceased injured person.

The Trust can only provide information about a claim to the authorized representative that appears on the claim form.

An attorney does not represent me; can I appoint another person to contact the Trust with questions about my claim?
The Trust cannot release information about your claim to anyone other than the authorized representative unless you appoint a "Contact" person. A Contact can retrieve information about your claim but cannot make changes to your claim. If there is a person that you would like to appoint as a Contact, request this in writing to the Trust.

What if my address changes while my claim is in the process of being evaluated?
You must notify the Trust either in writing at P.O. Box 1036, Wilmington, DE 19899-1036 or by email at helpline@celotextrust.com.
Can I change the option type of my claim after I have filed?
Yes. You must submit a request in writing. Supplemental medical and exposure information may be required.
I am an attorney representing an injured party, what should I do if my client is diagnosed with an additional injury after I have already submitted the claim form?
Provide the Trust with an amended page three of the claim form or the date of diagnosis and name of the new injury. In addition, supporting medical documents are required.

What does it mean if my claim is on "DV" or Definite Statute of Limitation Violation hold?
This means that your claim has been reviewed for statute of limitations compliance as outlined in Section 7.12 of the Claims Resolution Procedures and a letter was sent to you outlining why the statute was not met.
