Deficiency - 138 - Medical Reports are from an Unacceptable Screening Facility Explanation: The physical examination, chest x-ray, or pulmonary function test (PFT) were/was performed by an unacceptable screening facility. Instructions: The physical examination report submitted with the claim was performed by an unacceptable facility. Currently, reports from these facilities are not acceptable. Please submit a report that diagnoses the alleged injury from an acceptable facility . The complete report must be dated, signed by a qualified physician, and the diagnosis must be based upon a physical examination, or if you can demonstrate by physician affidavit that the unacceptable facility did not participate in the physical exam the Trust will re-review the claim.
