Detail Text: You have indicated that the injured party's exposure to Celotex/Carey Canada asbestos products began January 1, 1975 or after. On Part 4 of the Claim Form, please describe whether the injured party's employment and occupation included responsibility for the maintenance, rip-out, renovation, or repair of equipment or facilities where Celotex/Carey Canada products were used (see page 5 of the Instructions for Filing a Claim).
