Explanation: Chest x-ray report only identifies unilateral findings

Instructions: Please submit a report based on the review of a chest x-ray, CT scan, or a B-reader report indicating the alleged injury and evidence of bilateral findings. The report must be dated and signed by the radiologist or physician, and must include information identifying the injured person.

Detail Text: The chest x-ray, CT scan, or B-reader report submitted does not show bilateral findings or disbursement of findings. Please submit a report based on the review of a chest x-ray, CT scan, or a B-reader report indicating the alleged injury and evidencing bilateral findings. The report must be dated and signed by the radiologist or physician, and must include information identifying the injured person.
