Instructions: Please submit a more recent report based on the review of a chest x-ray, CT scan, or a B-reader report indicating the alleged injury. The report must be dated and signed by the radiologist or physician, and must include information identifying the injured person.

Detail Text: The most recent chest x-ray, CT scan, or B-reader report disputes an earlier report, and the most recent report does not provide an acceptable diagnosis or conflicts with another x-ray for the same date for the alleged injury. Please submit a more recent report based on the review of a chest x-ray, CT scan, or a B-reader report indicating the alleged injury. The report must be dated and signed by the radiologist or physician, and must include information identifying the injured person.
