Deficiency - 131 - Medical Report is Unacceptable

Explanation: Medical report does not provide an acceptable diagnosis for the alleged injury

Instructions: Please provide a medical report that documents the diagnosis of the injury alleged on the Claim Form. The report must be dated, signed by a qualified physician and must include a diagnosis based on a physical examination. Please do not send duplicates of previously submitted reports.

Detail Text: The most recent physical examination report, pathology report or medical records review does not provide an acceptable diagnosis for the alleged injury. Please do not send duplicates of previously submitted reports. Please provide a medical report that documents the diagnosis of the injury alleged on the Claim Form. The report must be dated, signed by a qualified physician, and must include a diagnosis based on a physical examination as required by the Trust's Instructions for Filing a Claim. A pathology or autopsy report is acceptable if it provides the appropriate diagnosis.
