Detail Text: The medical records provided in support of an alleged malignancy claim do not indicate that the alleged malignancy was the primary-site malignancy, as required in the Instructions for Filing a Claim form. In order for the Trust to be able to establish that the alleged malignancy was a primary-site malignancy, a physician's report expressly stating that the malignancy was a primary-site malignancy is required.
